The man had been carried in with what Sarah's estimated was a perforated bowel and a lacerated liver, both caused by something that had punched through his midsection with a force and geometry that didn't match any trauma she'd seen in seven years of MSF field work. She'd seen machete wounds in South Sudan. She'd seen propeller injuries from migrant boats in the Mediterranean. She'd never seen puncture marks arranged in a radial pattern like the points of a compass.

For a brief moment she had seen some sort of writing, but she shook her head and concentrated again. It was probably stress. The generators should have kicked in but they didn't. "Open the window and someone find a lighter or something," she said and kept working. The same way she'd kept working in a surgical tent in Juba when the generators failed during a mortar barrage. You didn't stop because the lights went out. You stopped when the patient died or you fixed them. Those were the two exits. Then the man died. It was morbid to think of it as anti-climactic but one moment he was breathing under her hands and the next moment he wasn't. It was the most sudden passing she had ever seen.

Twenty-five percent of the hospital went with him. Sarah counted the dead the way she'd been trained to count them in mass casualty events. You did it quickly, accurately and without pausing. Seventeen deaths in her ward alone. Patients, staff, visitors. There was no seeming pattern to it except that the blue lattice appeared on some and not others, and the ones without it were the ones who collapsed. The first night she worked eleven hours straight. No running water after the first hour and no communication with anyone outside the building. The violet sky outside the windows provided enough light to work by during the day, and at night they used torches and candles and the faint blue glow that came from the lattice under the survivors' skin. It was a building full of hurt people with no infrastructure to help them. Then the creatures appeared in the parking lot. She heard a scream and then the sound of something heavy hitting a car. She ran to the window and saw two creatures in the car park. They looked like dogs but weren't, and they were circling a man on the ground who was waving a tire iron about. His blood was bright against the tarmac. Sarah was outside in a flash, her MSF training kicking in. When someone is bleeding, you go to them.

You don't assess from the window. The creatures scattered when she came through the doors and she thought belatedly that running in without support was foolish of her. Then to her relief there was an orderly coming out behind her. The man's arm was torn open from elbow to wrist, the brachial artery nicked, and blood pulsing in bright rhythmic jets. She clamped the artery with her fingers putting direct pressure above the wound. She'd done this a few times to shrapnel wounds. It was the same technique.

The same steady pressure and the same voice in her head telling her to: hold it, hold it, hold it. And the Ether answered. The response came without decision or thought to her mind's urging. Energy moved from somewhere inside her chest, down through her arms, out through her fingers, and into the wound beneath her hands. The arterial jets of blood lost their urgency, the pressure dropping from lethal to manageable.

The tissue under her fingertips warmed, with the tissue changing at a level deeper than she could see. The wound's edges drew fractionally closer. In three minutes, the blood flow was a seep. In five, the seep was a weep. Her ten years of medicine told her that what she was seeing, what she was doing was impossible. "You kill things, you get experience. Enough experience, you level up. You get stronger. The blue text is tracking it - Integration is XP. The number after the slash is the threshold. When we hit it, we level. Like we did just now" The Baron looked at the dead creatures on the ground. Then turned at Enzo and said, "Explain it again. Slowly." Enzo explained. While Baron did not pay attention to all gaming terms he did understand the principle. And the principle was simple, violence produced power.

Killing creatures made you measurably stronger using these attribute points. The more you killed, the faster you grew. The blue boxes were not only tracking; they were rewarding it. The Baron walked to the edge of the dock and looked out across the Mediterranean. The island was small, and the creatures were everywhere. And every other survivor group was hiding behind whatever walls they could find, waiting for rescue that wasn't coming.

"Enzo. The creatures out there. How many would you say?"

"A few dozen? Maybe more around the temples. The old sites are crawling with them, something about those locations draws them in."

"And every one is worth experience? This integration?"

"If we can kill them, yes." The Baron was quiet for a moment. He was running the same calculation he'd run a thousand times in the smuggling trade: risk, reward, investment, return.

"So while everyone else hides," the Baron said, "we hunt. And every day the gap between us and them gets wider." He turned to his crew, he had thirty-two fighters, weapons, walls, boats, and a young man who understood the system because he'd spent his adolescence preparing for the apocalypse without knowing it.

"Get everyone up," the Baron said.

"We're going to the temples at first light. The creatures want to fight. So do we." He smiled up at the sky. It was the smile of a man who'd just realised that the rules of the old world were gone and the rules of the new one favoured exactly what he'd always been. * III. He came to the hospital five days later with four of his crew, all of them moving with the confidence of men who'd been fighting daily and could feel the difference in their bodies.

The Baron walked through the hospital's shattered entrance with the easy authority of a man visiting a property he was considering acquiring. He was bleeding. A gash across his forearm, deep enough to show muscle, wrapped in a strip of boat canvas that was soaked through. One of his crew had a similar wound across his shoulder. They'd been fighting near the temples. The creatures were getting larger and more aggressive but the experience was greater too.

The Baron had decided that acceptable casualties were part of the cost of accelerated growth. Sarah was in the emergency bay when he appeared. She'd been working for sixteen hours. Three patients that morning and two the previous night, she'd lost one because the damage had been too extensive and her reserves had been empty. She was running on the professional momentum that MSF had welded into her.

"Doctor, " Baron said when she was done. His voice was pleasant. Warm, even. The Maltese-accented English had the practised smoothness of a man who'd done business across six countries.

"I've heard you can do something remarkable." Sarah looked at his arm and the blood. Her hands unwrapped and examined the wound on autopilot.

"Sit down. The laceration is deep but the tendons are intact." She put her hands on the wound and the ether moved with the familiar warmth. The bleeding slowed and the wound edges drew closer. She held contact for ten minutes, feeling the tissue repair accelerate beneath her fingers. When she pulled away, the wound was half the size. It would be fully sealed by morning. A week's recovery compressed into hours. The Baron flexed his arm and tested the range. His face registered nothing except calculation."

How many people can you treat in a day?" The question sounded clinical and curious. Sarah answered it the way she'd answered a hundred administrative questions during MSF rotations.

"Depends on severity. Minor wounds, I can accelerate maybe eight before I'm spent. Ten, fifteen minutes each. Major trauma, three or four, and they need hours of sustained contact. I'm not performing miracles. I'm speeding up what the body is already doing. The patients still need rest, food, fluids. The healing is faster, not free." The Baron nodded. He looked around the emergency bay. There was no power, no water. Supplies dwindling.

"Doctor, how long can you keep this up here? I'm sure you realise this is untenable," She didn't have an answer. The hospital was dying. She'd been watching it die for four days and treating the symptoms while the structure collapsed around her. The Baron leaned forward. Conversationally. "I have a compound on the south coast. It has walls. Armed fighters and food supply for months. We've been clearing creatures around the temples and getting stronger every day, but my people get hurt in the field. A man lost a hand yesterday because the wound turned septic before we could close it." He paused. Let the image settle.

"You're running a hospital in a building with no power, no water, and creatures in the car park. I'm not asking you to stop what you're doing. I'm offering you a better place to do it."

"My patients -""Bring them. All of them. And your staff. And every piece of equipment worth carrying: beds, medicine, surgical tools, bandages, the lot." The Baron spread his hands.

"The way I see it, Doctor, you're treating the people of Malta from a building that can't protect them. I'm offering you one that can. You keep treating civilians, and my people join the queue like everyone else. Your mission doesn't change, just your location. The people would start showing up at the compound instead of here for treatment. I'll spread the word."

"And I help the people of Malta by helping you. It is a new world Doctor; we have to stand together" Sarah looked at his crew. They were armed and healthy and stood with confidence. The hospital was falling apart. She would have to abandon it sooner or later.

"This isn't a military arrangement," she said.

"I'm a doctor. I treat whoever needs it most, in the order they need it. Your people don't jump the queue because they're yours."

"I wouldn't dream of it." The Baron's expression was sincere. It was the most dangerous expression he owned.

"Triage is your department, Doctor. I wouldn't presume to interfere with medical decisions." To Sarah the math looked simple. Defensible walls plus medical supplies plus continued civilian access equaled more lives saved. When the field hospital is indefensible, you relocate to where the security is. That's not capitulation, that's operational logic.

"When?" she said.

"Tomorrow morning. I'll send men over to help with the move." The Baron stood and shook her hand. His grip was firm and dry and held hers for exactly the right amount of time. The handshake of a man who'd been making deals his entire life. Standing in a hospital that couldn't keep its patients alive, the Baron's offer was the most rational thing she'd heard since the sky turned violet. She went to find the charge nurse. There was packing to do.
